    How many have received an IRS Letter 5858 and what did you have to do?

    IRS received a copy of Form 1095-A for 2015. You must file your 2015 federal tax return with a Form 8962 to reconcile advanced premium tax credits.

    The IRS is sending you this letter because we received a copy of the Form 1095-A, Health Insurance Marketplace Statement, your Health Insurance Marketplace sent you. Form 1095-A shows advance payments of the premium tax credit were made to your or your family’s health insurance company to reduce your premium costs in 2015. To protect your eligibility for assistance with paying for your or your family’s Marketplace health insurance coverage in 2017, you need to file your 2015 federal tax return with Form 8962, Premium Tax Credit, to reconcile these advance payments as soon as possible.

    What you need to do
    1. Read your letter carefully.
    2. Review the situation to see if you agree with the information in the letter.
    3. File your 2015 tax return with Form 8962 as soon as possible, even if you don’t normally have to file.
    4. Use the Form 1095-A that you received from your Marketplace to complete Form 8962. If you need a copy of your Form 1095-A, log in to your HealthCare.gov or state Marketplace account or call your Marketplace call center.
    5. If you have already filed your 2015 tax return with Form 8962, please disregard this notice.
